In September, we sent out a copy of your student’s data to verify that the information is accurate and up to date. As part of a data review, we are sending an updated copy of your student’s Data Verification Form.  This form verifies your child’s personal information as well as your contact and emergency contact preferences. This information will be used as part of our family portal initiative, which will be implemented in the future. 

Please review the information, make appropriate changes, sign the bottom of the form and return to the school by March 28, 2024. The form should be signed by the family member or caregiver with lawful access to the student and will act as an attestation of the accuracy of the information. If you have had any changes to the information contained on the form such as contacts, address, e-mail, phone numbers etc.  If you have not had any changes to the information contained in the form you do not need to return the form.

To help confirm the data within the Contact Information, we’ve provided some definitions to clarify the meaning of the boxes within each contact.

Legal Guardian: This is the person that has LAWFUL decision making rights for the student.  This is often the biological or adoptive parent, unless noted by an Ontario Court Order.

Has Custody: A person who has custody of a child has the rights and responsibilities of a parent with respect to the child. If not the biological or adoptive parent, a court order is required to note custody of the student. Please note that notarized documents cannot transfer custody of a child. In sole custody situations, the Ministry requires the court documents or supporting legal documents to be retained in the OSR.  If changes are submitted it must be supported by legal documentation.

Lives with Student: This person lives with the student.

Pick Up Access: This person has permission to pick up the student from school through safe arrival or in an emergency situation as indicated on the registration form and/or DVF. Form IS-19-NG must be completed and signed by all individuals that have custody and guardianship of the student if there is an individual (eg. grandmother, grandfather, aunt, step parent, babysitter, etc.) who will be picking the student up on a regular basis or calling in attendance for the student. This form does NOT give access to education related information for the student.

Access to Records: This person is LAWFULLY allowed to have access to student records.
