Dear PHS parents, caregivers and students 18 and over,

On Tuesday, September 15th, student Data Verification Forms (DVF) will be distributed to students. This form verifies your student’s personal information, as well as address, and emergency contact preferences.  Please review the form for accuracy, and sign and return the form by Friday September 18th.  The form should be signed by the family member or caregiver with lawful access to the student and will act as an attestation of the accuracy of the information.

Your student should receive their DVF from their homeroom teacher. If your student has a spare first period, please have them pick up their DVF from the main office.
