Ensuring updated medical information is essential to the health, safety and well-being of all WRDSB students. If your child has a life threatening condition such as epilepsy, diabetes, asthma, anaphylaxis or any other life threatening condition, a Medical Plan of Care is required.

Students with pre-existing plans of care will receive their copy on Tuesday, September 15th. It is important that the school has updated information so that we know how to respond in the event of a medical emergency. The provided medical information will be kept on file in the office and shared with appropriate staff as needed. Please review the documents, and return to the school by Friday September 18th.

Once this information is received, the Plan of Care will be updated and a final copy will be sent home for your approval and signature. If your student requires a plan of care and one has not been set up previously, please contact the PHS Main Office.
