This is a message from Region of Waterloo Public Health

This month, Public Health is sending letters to parents/guardians, or students over the age of 16 with incomplete vaccination records. The Immunization of School Pupils Act (ISPA) requires that all students in Ontario receive certain vaccines or provide a valid exemption form to attend school.

If you receive a letter from Public Health, you may need to:

– Book an appointment with your doctor to receive required vaccinations
– Submit records to Public Health online at www.regionofwaterloo.ca/vaccines

If you receive a letter and have questions, please visit www.regionofwaterloo.ca/vaccines.

– View a translatable version of the letter you received
– Request or report vaccine records
– Submit vaccine exemption forms
– Book vaccine appointments
